A Clinical Review of Gastroduodenal Perforations

Abstract
This reports a clinical review of our experience with 90 cases of Gastroduodenal perforation which were treated surgically at the Soo Do Medical College Hospital from September, 1959 to August, 1963.
1) Gastric. ulcer perforation was the most common incidence involving 60 cases(66.7 % ), while the remaining cases included 12 cases(13.3%) of Duodenal ulcer perforation, 12 cases(13.3%) of Stomach perforation due to carcinoma, and 6 cases(6.7%) of Traumatic perforation.
2) The sex distribution assumed a ratio of seven to one with 79 males and 11 females. The age range in the majority of the patient was three decade of life(37.8%); the youngest was fifteen and oldest was 67 of age.
3) The seasonal distribution indicated a predominant incidence in Autumn and Winter(68.9 % ). The highest incidence of Gastroduodenal perforation occured in September(13 cases).
4) We have found the stomach to be a common site for ulcer perforation(60 cases) than the duodenum at a ratio of 5 to 1. The site of stomach perforation(78 cases) was examined at surgery. 33 cases(42.3%) of the perforations were pyloric, 40 cases(51.3%) were Stomach antrum and 5 cases (6.4%) were Lesser curvature in location. Among 12 cases of Duodenal perforation, 8 cases(66.6) were perforated within 1 cm. distal to the pyloric ring.
5) As for the operative fashion in our 90 cases: 82 cases were for simple closures and 8 cases for immediate gastrectomies.
6) The mortality was 3.3%(three cases) for all 90 cases and was highest for those who had a perforation which was treated with an immediate gastectomy(12.5 % ), where the patient was treated with simple closure the mortality was 2.4%.
7) X-ray demonstration of spontaneous pneumoperitoneum by simple plain was noted in 82 per Cent of the patients.
